Breast implant removal surgery: everything you need to know
Deciding to have your implants removed is a valid decision and, of course, there are many different reasons why you might reach this conclusion. After breast augmentation or a lift with implants, it is completely normal for your wishes to change over time. Some women feel that their bodies have evolved, that their priorities are different, or that they simply want to return to a more natural look. Others may experience discomfort, changes in shape, or simply the need to close a chapter. Whatever the reason, deciding to remove your implants is a valid and respectable decision, and explant surgery is here to help you.
As a plastic surgeon, an expert in female procedures, and with more than 20 years of experience, I will tell you everything about breast explantation, a surgery that is rarely discussed.
What is breast explantation?
Explantation is the surgery in which breast implants are permanently removed. Depending on the case, it may also involve managing the capsule that forms around the implant.
This surgery is usually combined with other procedures to improve the shape of the breast once the implants have been removed. Every explantation is different, as each patient’s body, the length of time they have had the implants, and the type of implant influence the planning of the surgery.
Unlike breast implants, this procedure does not seek to increase size, but rather to restore shape and achieve a more youthful and aesthetic appearance.
How is explantation surgery performed?
The surgery is performed under general anesthesia and the technique is adapted to the needs of each patient. In many cases, the same scars from the previous surgery are used to remove the implant.
During the procedure, the implants and the capsule that the body formed around them during the time the implant was in place are removed.
In some cases, a breast lift may also be performed to improve the shape and position of the breast tissue after the implants are removed. The goal is to achieve a harmonious, natural result that meets the patient’s desires.
It is also an option for those who have differences in height or shape, have experienced significant weight loss, and want to improve the shape without necessarily adding volume.
Who is an ideal candidate for breast implant removal?
There are many reasons for deciding to remove implants, whether for aesthetic reasons, lifestyle changes, or other personal decisions. Some patients also experience discomfort due to the size or weight of the implants, capsular contracture, or other situations that lead them to consider explantation.
The ideal patient is someone who has realistic expectations, understands the process, and is ready to embark on this new stage with clear information. During the consultation, we evaluate the condition of the tissues and skin, the size of the implants, and discuss the possible results.
You should keep in mind that when you decide to remove your implants, your breast volume may decrease by up to 90%.
Recovery after explantation surgery
Recovery is usually simpler compared to breast augmentation surgery, although each body responds differently. It is normal to experience swelling and sensitivity in the first few days, so I recommend complete rest, wearing post-surgical garments, and attending all follow-up appointments scheduled by the specialist.
Over the weeks, the body adapts to its new shape and the tissues begin to settle into place until the final result is achieved, which can take between 4 to 6 months.
How do breasts look after an explantation?
This is one of the most frequently asked questions. After removing the implants, the appearance of the breasts will depend on the size of the previous implant, how long it has been in place, and the quality of the skin and breast tissue.
In some cases, the breasts may look emptier or less firm, so a breast lift may be considered to improve the shape, or even liposuction to use the extracted fat to add a little volume and shape. My goal will always be to achieve a natural, harmonious result that is in keeping with each patient’s body.

Frequently asked questions about explantation
Does explantation leave new scars?
In many cases, I use existing scars, but you should know that it will depend on each patient. If an additional breast lift is performed, new scars may be created, which we will explain during the consultation.
Is a lift always necessary after implant removal?
Not always. It will depend on the quality of the skin, the previous volume, and the result the patient desires.
Is recovery painful?
It is usually manageable with medication and proper care. Each patient experiences recovery differently.
Can I have implants again in the future?
Yes, although many patients who opt for explantation are looking for a permanent result without implants. This is a decision that can be discussed during the consultation.
When will I see the final result?
The result evolves over the course of several weeks. As the swelling subsides, the tissues settle, and the breast takes on its final shape, it could take between 4 to 6 months.
